Form 1701
Annual Income Tax Return For Self-Employed Individuals, Estates and Trusts. This is used to file the annual income tax of individuals and entities.
Form 1901
Application for Registration for Self-Employed and Mixed-Income Individuals, Estates, and Trusts. This is used when registering with BIR. This contains the doctor’s information, their Tax Types, and the details needed for their service invoice.
Form 1905
Application for Registration Information Update/Correction/Cancellation BIR Form 1905 is used to update or make changes to existing Certificate of Registration. The most common applications include change in address, name and tax types. This is also used to close an active BIR profile and replace COR, ATP, and ID.
Form 1906
Application for Authority to Print Receipts and Invoices. This is accomplished whenever invoices and receipts need to be printed. This is filled in with the receipts, invoices, and authorized printer details. A draft of the receipt or invoice accompanies this document.
Form 2307
Certificate of Creditable Tax Withheld at Source. This is issued to doctors to certify the amount of income taxes withheld by the payor on income payments.
Form 2316
Certificate of Compensation Payment/Tax Withheld. This is issued to employed doctors as a certificate of the income earned and taxes withheld during the year.
Form 2303
Certificate of Registration. This is issued by the BIR as proof of a doctor’s registration and contains their TIN and registered activities.
Routing Form
A form used to track the progress of documents as they move through different departments or individuals.
Form 0605
Payment Form. This is used for the payment of taxes and fees that do not require a tax return, such as annual registration fees.
Form 1701Q
Quarterly Income Tax Return for Individuals, Estates, and Trusts. This is used by self-employed doctors to report income and pay quarterly income taxes.
Form 1701A
Annual Income Tax Return for Individuals Earning Purely from Business/Profession (Optional Standard Deduction) This is filed by doctors to report their annual income and deductions, using optional standard deduction.
Form 2551Q
Quarterly Percentage Tax Return This is used by non-VAT registered doctors to file and pay the percentage tax due every quarter.
Form 2550Q
Quarterly Value-Added Tax Return This is used by VAT-registered doctors to declare and pay the value-added tax (VAT).
